Funded by Liverpool CCG, this project has a unique patient pathway whereby GPs who identify patients at risk of social isolation will directly refer onto the project. The delivery includes reminiscence sessions, visits to local places of interest and interactive sessions at Goodison Park. This is the first time this patient pathway been established.
